00 (depending on experience) Work Duties Provides support for Health Center admission policies and procedures in cooperation with the Health Center Administrator to meet company standards and governmental regulations.
Assists with intake functions, admission contracts, creating files, assuring room readiness, tours, and other Facilitates tours and helps to assist team members in environment in always being tour ready, Receives inquiries by use of current referral software, phone, fax or e-mail.
Plans /coordinates marketing events with members of the health center sales and marketing team.
Notifies nurses on the floor of admissions Sends out a daily bed availability to hospice agencies and other referral sources.
Adheres to HIPAA privacy rules and company policies, procedures and practices which require that resident-identifiable health information will only be used to perform the essential functions of the job; Compiles reports regarding referral statistics, ensuring accuracy and integrity of referral data; Ensures financial verification - verifies known status of Medicaid,/ Medical funding determining if in pending status or whether application initiation is needed.

Identifies marketable aspects of SNF program and events happening in full LPC, supporting marketing collateral development and deployment tactics.
Maintain working knowledge of, and assist in, accessing community resources; Attends in-service training workshops and meetings as required; Provides effective and courteous service to all residents, guests, and co-workers; Promotes and protects the rights of each resident; Performs other related duties as required.
Implement and monitor wellness and recreation programs to meet the specific needs of residents Support programs and events that encourage engagement and socialization to help support high quality of life for our residents Supports, directs and participates in variety of activity programs for residents, such as entertainment, cooking, educational programs, arts and crafts, games, sports, and outings; 1:
1 activity will also be provided to support residents that choose not to engage in group activities Coordinate transportation of residents to and from events outside of the community, which may include driving a community motor vehicle to transport residents to and from activities and outings in a company vehicle Support and direct volunteers as appropriate Work closely with the Activity Coordinator - SNF to implement the activity plan as developed As the largest nonprofit owner/operator of senior living communities in California and one of the largest in the country, we are more than just a place to work.
